As the email you received stated, you will have several different appointments for your physical, PPD, drug screen, and Human Resources Paperwork.   As a reminder, you cannot miss or rescheduled your physical and drug screen appointment.  Below is a reiteration of the items you need to complete prior to your Paperwork Appointment:

- FBI Fingerprints
- ACT 31 Training
- Relias Documents (Training Sign-Off)

Required Documentation for Human Resources Appointment
Below are documents you are REQUIRED to bring with you and provide during your Human Resources appointment provided in the email. 
 
If you report without any one of the following items, we will need to send you home and reschedule your appointment date and possibly start date.  Please note that we must see the original versions of these documents.  *If you do not have any of the required items below, please call/email us immediately.
 


Please review the benefit information provided below.  During your new hire paperwork you will make your choices for insurance enrollments.

  •  I9 Employment Eligibility Documents.  Click Here For List Of Acceptable Documents
  •  Original Diploma or Official (sealed) Transcripts of your Highestdegree earned (if your position requires a certain diploma level or if your pay rate is based on a certain diploma level we must see a diploma at that level or higher).
  • Direct deposit information (please bring a voided check or documentation from your bank with routing and account numbers).  Direct Deposit is an agency requirement.
  •  Social security cards and birthdates of dependents for insurance enrollment.
  •  Any certifications or licensure required for your position (social work, nursing, education, etc)
  •  Any current CPR or First Aid certifications you may have.
